REFERRING TO POSITIVE NUTRITION

Thank you for entrusting us with your clients and patients. We will do our best to take great care of them. This page explains how to make a referral to our practice, ensuring we have all the information we need to smoothly bill their insurance and have all the information we need to get started.

For all new clients, we need the following:

1. The diagnosis form faxed to us from the referring provider (click here for therapist referral form and click here for medical provider referral form)

2. The client to fill out the request form.

That's it, we'll take it from there!

It is not in a registered dietitian's scope of practice to diagnose. Therefore, when we bill insurance under a medical or behavioral health diagnosis, we require documentation of the diagnosis to ensure we are complying with licensure rules and our scope of practice.

If you are a therapist referring to our practice, please fill out this form and fax it to our office. If you are a medical provider and referring to our practice, please fill out this form and fax it to our office.

In addition to faxing us a diagnosis, please also send the referred client/patient to our Request an Appointment page of our website. That's where we gather the information necessary to match them with the right RD in the practice.
