  • If you plan to use your insurance, you are responsible for understanding your benefits​

  • Documentation of medical and/or eating disorder diagnosis is typically required—please have referring provider fax us this form

  • Most plans cover diagnosed eating disorders, medical diagnoses such as diabetes etc., and some do not require diagnoses (can bill under preventive)

  • Telehealth is usually covered—you can call your insurance company to find out

  • Please call the number on the back of your card and use this script to verify your benefits
