If you have questions about our philosophy or what to expect for working together, please find the answer to your question in the FAQ section. We accept appointment requests via the button below, and do not make appointments over the phone. We have self-pay rate information and information about insurance on other pages on this site.
If you are a returning client, you can access the client portal here.
If you would like to use your insurance for your nutrition therapy appointments, please call the number on the back of your card and use this script to verify your benefits. Please also ask your doctor or therapist to fill out this form and fax us documentation of your diagnoses so we can bill your insurance properly.
CREDIT, CASH, CHECK ACCEPTED
Payment is due at the time of service for all self-pay clients. For your convenience, we accept many forms of payment including major credit cards, HSA, FSA, check or cash. All clients are required to have a credit card on file to reserve appointments.
Please note: we require at least 24 hours notice to cancel or reschedule appointments. In the case of a no-show or late cancellation for an appointment, you will be billed for the entire hour. A credit card will be required to hold your appointment spot; please do not make an appointment unless you plan on attending.

Even if we don't contract with your insurance, your visits may still be covered by your insurance carrier. If you'd like to submit your visit with us to your insurance for possible reimbursement, be sure to ask for a superbill at your appointment.
